Programs, eligibility, and who may benefit.
Remote Patient Monitoring (RPM)
For eligible Medicare Part B patients who may benefit from home monitoring for hypertension, diabetes, CHF, COPD, weight management, or oxygen levels.
Chronic Care Management (CCM)
For eligible Medicare Part B patients with two or more chronic conditions who need ongoing coordination, care planning, and monthly support.
Behavioral Health Integration (BHI)
For eligible Medicare Part B patients who benefit from behavioral health care coordination and closer connection to primary care.
Developmental Disability Services (Coming Soon)
Compassion First is pursuing Virginia DBHDS licensure. Future support will be available to eligible individuals with a Virginia Developmental Disability Waiver, including Community Engagement and Community Coaching.
